Service Request for 3 Inches and Smaller

The applicant must submit a completed SJWC Form 40 Questionaire along with a site plan and/or improvement plans for the project. |
|
The service is sized, backflow requirements are determined, and a construction agreement is sent out to the applicant with a cover letter that explains the procedure to be followed to process the service request for installation. |
|
An Affirmative response will initiate the preparation of an Service Installation Order (SIO) or Orginal Meter Install (OI) |
|
These forms are forwarded to the SJWC Construction Department for scheduling of the installation. |
